To determine insurance eligibility, simply call (800) 759-1930 and provide your insurance information to our friendly staff, 24 hours a day 7 days a week. You may also enter your information under the "Insurance Verification" form below. Results will be communicated to you no later than 48 hours upon submission.



  1. Those who will be flying to our facility can make arrangements to land at one of the following airports:

  2. We encourage new clients to schedule their arrival date on Sunday.
  3. Travel Checklist:

    • casual clothes (flip flops, sandals, comfortable pants, shirts, sweater)
    • active wear (i.e. sweat pants, leggings, workout attire, running shoes, socks, face towels)
